ARCTIC CONVOY STAR MEDAL APPLICATION BY NEXT-OF-KIN
It has been advised by the Medals Officer, MCA, Cardiff that an application by a next-of-kin for this medal or any other medal(s), must be accompanied by a Medals Statutory Declaration form which is attached. The Arctic Star application form and the application guidance for the medal is also attached.
If the person to whom the application refers, was presented with the Arctic Emblem or the Soviet 40th Anniversary Medal, no further ship evidence is required. If not, copied evidence will be required (even for Atlantic Star Medal holders) of ship(s) served on during Arctic Convoy voyages and the appropriate Ship Journey cards.
